This is the current disclosure shown with the text-message box on your child's assessment. The exact wording you agreed to is preserved on your consent record at the time you opted in.

By checking this box, you agree to receive text messages from KiroKids Connect at the mobile number you provided about your child's assessment with your child's healthcare practice: a link to your child's health report when it's ready, and follow-up and re-check visit reminders. Message frequency may vary (typically a few per year). Standard message and data rates may apply.

Checking this box is not a condition of completing this assessment or of receiving care. Reply STOP to opt out. Reply HELP for help. STOP ends all texts from KiroKids Connect.

We will not share your mobile information with third parties for promotional or marketing purposes. Text messages are not encrypted and may be visible to anyone with access to your phone; by opting in, you accept this risk.

The text-message checkbox sits directly under the phone number field on the child health assessment form (in-office tablet or web). It is unchecked by default, separate from every other agreement, never required to complete the assessment, and the full disclosure above is shown inline with it.

Your mobile number and text-message opt-in status are used only to deliver the messages described above. They are not sold or shared with third parties for their marketing purposes. Text messages are not encrypted and may be visible to anyone with access to your phone; message bodies never contain your child's name or assessment results — the report itself lives behind the link.
